People who have Diabetes, Cardiovascular disease (including hypertension, congestive cardiac failure and angina), asthma and Musculoskeletal conditions would receive help.
Services will begin on 1 July this year and will be delivered by Moyne Health Services, South West Healthcare, Beaufort & Skipton Health Service, Heywood Rural Health, Maryborough District health Service and East Grampians Health Service
