4 Places allocated, STRC is aimed at reversing and/or slowing 'functional decline' in older people with the aim of improving wellbeing. It seeks to achieve this through the delivery of a time-limited (up to 56 days), goal-oriented, multi-disciplinary and coordinated package of services that are designed for, and approved by, the care recipient. STRC may be delivered in a home based setting, residential based setting or a combination of both.
